The We Grow Together! Coalition believes that relationships are the foundation of a healthy community system.


Our model for a healthy community system is a network of empowered relationships guided by fair and transparent policies. In our model, this network of relationships is centered in physical locations called Community Health Hubs (CHHs), where people can gather to access resources, engage in healthy activities, learn skills, and spend meaningful time with one another. CHHs are place-based and community driven which means that choices, vision, and leadership all happen at the neighborhood community level.

 

Through Community Health Hubs we promote strong relationships between:

  • We need clean air, clean water and healthy soil for communities to be sustainable for future generations. At CHHs, we grow awareness and advocate for these concerns.

  • Organizing CHHs is a powerful way to eliminate the barriers to having nutritious food. Food access barriers include financial, physical, knowledge, and cultural barriers. Community members come together to obtain, share, learn, and advocate for nutritional food.

  • All of the resources available at a CHH - physical, educational, health-promoting, and social - work together to enhance each individual's experience of a healthy, meaningful, and fulfilling life.

  • A sense of belonging to a community motivates us to commit time and energy towards the vision of a healthy, sustainable future. Our relationships with one another are guided by the principles of diversity, sustainability, inclusiveness, empowerment, resourcefulness, justice and consensus.

The Community Health Hub model is easily adaptable to different locations, circumstances, and communities. It is resilient in response to change, and it is empowering by supporting people in meaningfully addressing important issues. It allows people to participate and gain benefits at whatever level of involvement is comfortable for them.
